Heparin Drip Calculator
Calculate heparin bolus dose, infusion rate, and aPTT-based titration for DVT/PE and ACS protocols.
⚠️
HIGH ALERT MEDICATION -- Heparin requires independent double-check by two nurses. Always verify weight, protocol, and concentration before administration.
aPTT Titration Guide (DVT/PE Protocol)
| aPTT (seconds) | Action |
|---|---|
| < 35 | Re-bolus 80 u/kg, increase rate by 4 u/kg/hr |
| 35 - 45 | Re-bolus 40 u/kg, increase rate by 2 u/kg/hr |
| 46 - 70 | Therapeutic -- no change |
| 71 - 90 | Decrease rate by 2 u/kg/hr |
| > 90 | Hold 1 hour, decrease rate by 3 u/kg/hr |
TIP
Always use your institution's specific heparin protocol. Titration tables vary between facilities. Check aPTT 6 hours after any dose change. Heparin is a high-alert medication -- independent double-check is mandatory.
